My system locale is ru_RU.koi8r. After latest update there are no problems in the console and GTK file dialogs, however I met ptoblem in X terminal emulators and Qt file dialogs. Cyrillic paths are displayed with questions in virtual terminals despite of charset settings. And cyrillic paths are not displayed at all in Qt file dialogs.

What you describe makes it sound like your emerge --info's LC_ALL=ru_RU.koi8r isn't exported in your desktop environment.

On a fully updated ~arch system I've randomly tried to `LC_CTYPE=ru_RU.koi8r konsole` (encoding set to koi8-r) and `LC_CTYPE=ru_RU.koi8r ark` (kde-apps/ark) for its file dialogs and it displayed files with same encoding just fine.

If I don't set LC_ALL or LC_CTYPE, then I get ???? in konsole, and non-displayed files in ark's file dialogs.

Or perhaps broken/missing? Is running `locale-gen` giving any errors?

locale-gen works without errors:
locale-gen

 *  (3/3) Generating C.UTF-8 ...                                                                                                                                                                                                  [ ok ]

When I run xterm from xfce4-terminal I see in its environment only LC_MEASUREMENT=C, LC_ALL=ru_RU.koi8r and LC_NUMERIC=C.

It is sad to read that you have problems with the software. The situation seems to be a bit more complicate and requires some analysis.
We can not help you efficiently via bug tracker. The bug tracker aims rather on specific problems in .ebuilds and less on individual systems. 

I have had very good experience on the gentoo IRC [1] with questions like this. Of course there are also forums and mailing lists [2,3].
I hope you understand, that I will close the bug here therefore and wish you good luck on one of the mentioned channels [4].
Please reopen the ticket in order to provide an indication for an specific error in an ebuild or any gentoo related product.
