This looks like the very same issue as bug #491560.

It also looks SIMILAR to bug #488492, which is triggered by kernel 3.12, and I see you and the reporter of 491560 are both on kernel 3.12.  This is just happening very intermittently, and only on kernel 3.12, right?

I believe this was caused by a bug in kernel 3.12 and 3.12.1
