perl-cleaner will fail if some Perl modules were installed with g-cpan.
AFAIK, this is basically bug #333267, which has been filled 4 years ago.

I tried app-admin/perl-cleaner-2.7 (the current unmasked version), 2.8 and 2.12.
All block at the same point.

# perl-cleaner reallyall
...

 *   Adding to list: perl-core/ExtUtils-Install:0
 *                   virtual/perl-ExtUtils-Install:0
...

These are the packages that would be merged, in order:

Calculating dependencies -

emerge: there are no ebuilds to satisfy "perl-gcpan/Object-Realize-Later:0".
 * perl-cleaner is stopping here:
 * Fix the problem and start perl-cleaner again.

So where did perl-gcpan/Object-Realize-Later:0 go? If the ebuild was removed from your overlay, then you should probably unmerge that package and run perl-cleaner again.
