Finally I stumbled across an obscure bug on debian about a missing libvpopmail.so causing the same issue. sure enough, nahda! This can only be treated as a doc bug - take a look at the ebuilds - even 0.60.6 has a hard fail on vpopmail useflag.

Nothing the GDP can do about it. If you look at the very beginning of the guide, there's already a big DRAFT disclaimer on it. It's not an official doc. Reassigning to the vpopmail/qmail maintainer/team. The courier-authlib ebuilds need fixing as upstream stopped supporting vpopmail. Mike, since this is your guide, you'll need to make any changes, like switching to dovecot or whatever. None of the GDP knows about that. Otherwise, we'll have to slap an OBSOLETE disclaimer at the top, and/or prune the guide if it's written for broken/nonexistent packages.
So obviously there is no bug at all, vpopmail is just deprecated / not supported by courier any more
