After the upgrade from 4.2.1 all menus and file associations are missing.
After a second sync I found a new dependency to xdg-utils which I emerged, nothing changed even after reboot. 
This might be related to bug 264647

please attach your emerge --info
as well logout from KDE session and in linux terminal run:

$ echo KDEDIRS
$ echo XDG_DATA_DIRS
$ echo XDG_CONFIG_DIRS

Imho, you clearly are not using the latest kdelibs and kdebase-startkde from portage, you can try reemerging them now anyway.

I found that the problem is the /etc/xdg/menus/kde-4.2-applications.menu

It should be applications.menu but does get changed to the other name.
Once I produced a symlink

lrwxrwxrwx 1 root root    25 14. Apr 15:31 applications.menu -> kde-4.2-applications.menu

and run kbuildsycoca everything works again

I did a sync on Sunday and re-emerged every kde build and also did another sync today and reemerged kdelibs!

You were correct that my start-kde was missing a change in the kde4patch. I will check if it works after that.

Yes, startkde script provided by newest kdebase-startkde sets XDG_MENU_PREFIX, and is required to pick kde-4.2-applications menu.

This is not a bug.
If you are impatient enought to check out uncommited kde then expect problems.
When i was finished with the commit i unmasked it.
Everybody whom used kde from that time should have everything working just fine.
