Output of 'emerge info' and 'uname -a'.  Please try with vanilla kernel ?

Quick question, why is /dev being populated with a ton of files if gentoo uses [and requires] devfs?  Nothing is required in /dev at boot except for /dev/console and /dev/null if I recall correctly.

Two fold:

 1) Not everybody let the kernel mount /dev

 2) Lets call it and further emergency effort .. if you need to boot without
    devfs.

Do you 'mount --bind /dev /mnt/gentoo/dev' or something before chroot ?

This is fixed in baselayout-1.8.6.6.

  Some of the build images have MAKEDEV in /usr/sbin, and we called it with
  /usr/sbin/MAKEDEV, which is wrong since we moved it to /sbin.  This caused
  things to break, as we called /usr/sbin/MAKEDEV, and subsequent calls used
  the copy in /sbin, which is different versions.  Add a symlink in /usr/sbin
  to overwrite the invalide MAKEDEV, and call it via /sbin/MAKEDEV.  This
  should close bug #15299.
