Hi, here is an ebuild for the Quake 1 campaign, "Zerstorer - Testament of the Destroyer", which features new weapons, monsters and items. The campaign maps are both single-player and multiplayer-ready, with additional multiplayer-only maps.

This mod is frequently reviewed as one of the best Quake 1 campaigns ever - some reviews are:

The ebuild was created by lazy_bum, and tidied by me.

The download is 12mb (surprisingly small).
