Unfortunately, this problem is reported in different distros and upstream with people reporting it still happens in 6.5 (the next kernel as of this writing), also. Not sure if you have anything new to add to that bug, but we'll keep an eye on it for anything we can potentially backport. It's been years since I've used a card of that vintage, but I recall the dynamic power management implementation being especially flaky on them. Might be worth trying with static PM or none at all.
