Same thing happened to me about a year ago with Qt 5.14. Thanks to asturm, issue was resolved. Here's the link on the forums:

However, with the release of Qt 5.15.2, this issue appeared again. Exactly same sympthoms, but the patch proposed by asturm doesn't work with this version. 
Downgrading to Qt 5.15.1-r1 helps.

Reproducible: Always

Steps to Reproduce:
1. Upgrade installed dev-qt packages (including dev-qt/qtgui) to 5.15.2
2. Launch any KDE app (e.g. Dolphin, Kwrite or System Settings)
3. Behold the fonts

Actual Results:  
Nasty upscaled and blurry fonts

Expected Results:  
Cute tiny and properly scaled fonts

Here you can see the difference. There were no changes in settings etc, just upgrade of those dev-qt packages.

There has been no other report of this kind in Gentoo and I have not seen anything similar on any system of mine.

The patch from January went upstream into 5.14.1 and there have been only two commits touching the file since our fix, both are already in 5.15.1:

Which means this is a different issue. Please try to reproduce your problem with a fresh test user profile. Then, please go directly to upstream (bugs.kde.org) with as much information as possible, starting with X11 vs. Wayland session behavior, any scaling settings you have in systemsettings, any nondefault settings you made in X11 config, and of course the fact that 5.15.2 behaves differently to 5.15.1.
