When trying to boot from a dm_cache backed partition, the initramfs needs to contain the drivers for it, the generated initramfs partition didn't contain the compiled drivers.

Please add to the list: 
 - dm-cache
 - dm-cache-mq
 - dm-cache-cleaner ( not sure if this one is really needed )

dm-cache dm-cache-smq are present in v3.5.2.x.
