Seems like this is a very common error, as per the bug report here: https://bugs.launchpad.net/ubuntu/+source/grub2/+bug/1274320

Comment 34 seems to confirm a fix for mdadm and lvm. However, the code offered to be replaced in the 00_header file doesn't exist in that file in Gentoo (at least) and not sure if this needs to be added or not.

Seems to fix the issue (not fully tested yet, will report back), but needs to be updated in two places to execute grub2-probe instead of grub-probe

What do you have GRUB_DEFAULT set to in /etc/default/grub?

What is the content of /boot/grub/grubenv?

Also, do you have GRUB_SAVEDEFAULT set?
