Example setup:

DVI-0 connected primary 1280x1024+0+0
VGA-0 connected 1920x1440+1356+0

Now if I have a window on primary screen and maximize it, it fits fine. However, all the dialog windows appear in the middle of virtual screen rather than in the middle of the window. In my case, it means that they appear somewhere on the second (larger) screen.

Could you please provide some additional information, including but not limited to any application in particular, video drivers, Qt 4 or 5 etc.

net-irc/quassel -- now that I test it more, it seems to happen only to some windows (e.g. settings, joining a channel). qt4.

This should definitely go upstream, unless you have a patch and you're asking for its inclusion in gentoo.
