The package mail-filter/spamass-milter lacks IPv6 support. But there is a patch available that Debian an Fedora have been using since 2010:

At least the patch "spamass-milter-0.3.2-rcvd.patch" should be applied as well, otherwise you'll run into the following bug: http://spamassassin.1065346.n5.nabble.com/sa-with-spamass-milter-UNPARSEABLE-RELAY-problem-td58397.html

I'm now applying all 7 non-Fedora-specific patches to the ebuild in my overlay (via epatch):
