The contents of ~/.Xdefaults:

Xcursor.theme: gentoo
Xcursor.size: 32

When KDE is loading, the cursor is the proper size of 32 while the splash-screen is visible.  Once the startup of KDE is complete, the cursor is returned to the default size (small).  If I click the 'Logout...' item in the KDE menu, the cursor size is once again 32 (while the logout/cancel dialog is visible)

I have mine stored in the ".icons" directory.  I'm guessing that KDE isn't respecting the Xsettings - but you'll need to talk directly to the kdevelopers about that.

If I am not mistaken, the ~/.icons directory is where the themes themselves are stored.

As for the second part, my response is that I'm unsure whether what you're describing is a bug or not.  If you feel it should work, then please report it upstream.

The link you provided shows how to install cursor icons.  This has nothing to do with setting the size of the cursor, only the images used.  My problem is a bug that doesn't honor the settings in ~/.Xdefaults while KDE is running (other than during the splash-screen and logout).
