This mod is now at version 0.12 with helpfuul configurations directives

It would be nice to upgrade the ebuild
It seem's only needed to :
  update version
  add a note about configurations directives change

There seems to be a 0.12 ebuild already?

It is just still prefixed. Add =www-apache/mod_xsendfile-0.12 to /usr/portage/package.keywords and you can install it via emerge.
