If you look here(ftp://ftp.idsoftware.com/idstuff/quake3/linux/), you can see that the current ebuild uses a pointrelease from november 16th, 2002. Since then there have been 2 updates, a -2 version(9th sep 2003) and a -3 version(2nd okt 2003). 

The only change I think is neccessary is to change this line in quake3-1.32.ebuild from

SRC_URI="ftp://ftp.idsoftware.com/idstuff/quake3/linux/linuxq3apoint-${PV}.x86.run"

to
SRC_URI="ftp://ftp.idsoftware.com/idstuff/quake3/linux/linuxq3apoint-${PV}-3.x86.run"

Ok, I tried installing it and noticed that the binaries are in a different place now, so the following line needs to be changed from

doexe bin/x86/{quake3.x86,q3ded} ${FILESDIR}/startq3ded

to

doexe bin/Linux/x86/{quake3.x86,q3ded} ${FILESDIR}/startq3ded
