Step 4 should suggest a size of 150MB for / (root).

Before Step 6, vgscan needs to be run otherwise none of the lvm steps will work.

If swap is going to be part of the lvm in Step 9, then Step 8 should mention 
that it needs to be created and Code Listing .4 should mention it.

Correct. This is already in the handbook draft, but should be added to lvm.xml
as well. I'll take care of that.
