When emerging Sendmail it is built without IPv6 awareness, even though the 
IPv6 USE-flag is set. Adding -DNETINET6 to confENVDEF allowed me to built the 
Sendmail package with IPv6 support.

Here's a diff with my changes which allowed me to built the Sendmail package 
with IPv6 support:
