I don't know why gtkstyle is working fine for hplip but not for k3b, k9copy or okular (all of them with its qt4 versions) 

I attach a screenshot showing the problem

hplip is a pure qt4 app, while the others are kde apps, so you need to configure their style in kde's systemsettings.

Is there any "manual" way to change it without needing to install kde-base/systemsettings ?  I mean, could I simply edit a configuration file or similar?

At least I found what needs to be changed for using gtk+ style by default without needing to ask every user to change it using systemsettings. 

You need to create /usr/share/config/kdeglobals with the following content:
[General]
widgetStyle=gtk+

I reopen this bug report as an enhancement request for asking you to add that information to qt-gui ebuild since, I think, most if users that merges it with "gtk" USE flag expect to get gtk style also with kde apps

It's a KDE configuration issue that has nothing to do with qt-gui itself.

But there are people that will install some kde apps (like k9copy or k3b) but won't install all KDE
