Reading the upgrade guide makes me believe it is wrong. The guide is placed here:
http://www.gentoo.org/doc/en/gcc-upgrading.xml

1. Point 4.5 "Code Listing 4.5: Cleaning up" tells me to unmerge the new version. This must be wrong. Also one might advise the reader to use the prune option as well.

2. At the end of 2.1 is written: "To be completely safe that your system is in a sane state, you must rebuild the toolchain and then world to make use of the new compiler." One should better point out that in most cases this isn't necessary. There might be problems, yes. But as long as the ABI doesn't change always doing an emerge -eav world is just a wast of time an natural resources such as energy.

For 1:
I see the point and still I think it would be much better to write it in a different way. It is placed in the section for fresh install which will most likely also be important for people new to gentoo. They might misunderstand it as well.

For 2:
There is now answer to this point why the bug should be reopened therefore.
