XSendFile is an extension for apache that processes X-SENDFILE headers registered by the original output handler. The homepage obviously explains it in more detail, but it seems to be an interesting extension to have for web services that need to hand out files after doing some initial checking (such as authorization).

I have a working copy of an ebuild in my overlay (called 'graaff' in layman). I'm including it here for easy reference, it may need a bit of work. I've tested it on amd64 and x86.
