Mirth is an open source health care integration engine. By supporting numerous standards (like HL7, EDI, XML, and NCPDP) and protocols (like LLP, JDBC, and FTP), Mirth allows for the filtering, transformation, and routing of messages between disparate systems to allow them to share data.
The latest version includes DICOM support, which really makes this thing look promising. 
To give the 'flavor' of the project, here's a quote from the main page: 'Linux had Apache, healthcare gets Mirth. Our "killer app" has arrived...'

I've done some searches, but I don't see an ebuild anywhere. 
Even an unfinished one that might need some testing would be great.
If I end up trying to make one myself, it would probably be very non-gentooish.
