I've experienced this on every machine I've upgraded from gnome-2.18.x
to 2.20.x so far. The applet cannot be added or dies and gets dropped
from the panel if it was already attached by a previous (working)
version of GNOME. All other applets I'm using seem to work correctly.

The error message is: "User Switcher" has quit unexpectedly
