The (not-so-trivial) workaround is, of course, to create all combinations of uppercase/lowercase letters in the file-extension entries. Could you please clarify why's this a Gentoo bug, as opposed to a feature request that belongs upstream? Please file an upstream bug at http://bugs.kde.org and post its URL here.
