I think there is a typo in rc script of mysql 3.23.56
(/etc/init.d/mysql) the script tries to run /usr/bin/mysqld_safe
instead of /usr/bin/safe_mysqld. I've marked this bug as uncorfimed
because i've upgraded mysql to 4.0.12 and then downgraded to 3.23.56
so i'm not really sure about that, anyway it could be nice to check

maybe the /etc/init.d/mysql you're seeing is from 4.0.12 (which uses "mysqld_safe"), and after 
downgrade you didn't etc-update that file.
