Durint the gentoo installer, there is a prompt to add new users. The user added will fail during install with the ERROR! message:
ERROR! Failure to add user <username>

Reproducible: Always

Steps to Reproduce:
1. Start the GUI installer
2. Enter an additional username
3. Watch the installer crash miserably

Actual Results:  
The user is not added

Expected Results:  
The user should be added

were you adding the user to any groups?  perhaps groups that didn't exist?
