I had the same problem; it cleared up after I emerged  app-text/docbook-sgml-utils with the tetex USE flag.

Hmmm..  Just saw that you _do_ have the tetex flag.  Perhaps re-emerge docbook2X and docbook-sgml-utils?

reemerge app-text/sgml-common, app-text/docbook-sgml-dtd, and eventually scrollkeeper and get back to us.
