Out when I try to emerge qmail-mysql.  I looked in package.mask but can't find 
anything that should stop it from installing.

mail profiles # emerge -p qmail-mysql
 
These are the packages that I would merge, in order:
 
Calculating dependencies   
!!! all ebuilds that could satisfy "qmail-mysql" have been masked.
 
!!! Error calculating dependancies. Please correct.

Steps to Reproduce:
1.try to emerge qmail-mysql

Actual Results:  
mail profiles # emerge -p qmail-mysql

Expected Results:  
Would have been nice if it compiled and installed.

Since the normal qmail ebuild no longer has any mysql options in it,  as far as 
I can tell it is not possible to compile qmail with mysql support.
