We won't add a USE flag for this for now, as it's not used by many people (you're the first ever to mention it), and the PHP developers discourage it's use in production, see [1]. If you need this, just do: EXTRA_ECONF="--enable-zend-multibyte" emerge php and it will emerge PHP while passing that configure switch, resulting in the BOM stuff enabled for you.
