My /var/log/Xorg.0.log includes the line
(II) Initializing built-in extension XINERAMA
even though I have USE=-xinerama set in /etc/make.conf. This happened with earlier versions as well, though I didn't think to look for it until now.

That's because xorg-server doesn't have a xinerama USE flag, and I have no interest in adding one for a couple of reasons. Some core X packages break if built without xinerama, and if someone wants to enable it later, it's not clear that they would need to rebuild xorg-server to get other programs working properly. It's also not well-tested by the upstream developers to avoid building such core options.
