If you are using Apache 1, execute:
# cp /usr/lib/mod_survey/survey.conf /etc/apache/conf/addon-modules/99_mod_survey.conf
# echo "Include conf/addon-modules/99_mod_survey.conf" >>	/etc/apache/conf/apache.conf

If you are using Apache 2, execute:
# cp /usr/lib/mod_survey/survey.conf /etc/apache2/conf/modules.d

Now restart Apache and test your installation: http://YOURHOST/mod_survey
