Since the upgrade to kde plasma 5.11.5, bold does not appear in syntax highlighting with monospace fonts, for example in Kate. It is unclear what is the culprit. There is an upstream bug that seemed to fit (https://bugs.kde.org/show_bug.cgi?id=378523), but the upstream developers say this is a different issue. It may even be a Gentoo-specific issue. It may be interesting to know if there are people with a stable Gentoo plasma install that do not have this bug.
I firmly believe this is the same bug as I had experienced it myself with syntax highlighting. But I did not experience 'bold' fonts problem with all fonts. Monospace (Liberation Sans Mono) worked fine, Go Mono did not and required kateschemarc workaround.

Qt 5.9 fixes this.
