In my system I've set the following default options.
/etc/portage/make.conf:
EMERGE_DEFAULT_OPTS="--ask --verbose --autounmask=n"

As example of fetch-restricted package I use sqldeveloper (see bug #525244 for ebuild).
Start point: package in installed (for new installation behaviour is exactly the same, tested on =dev-db/sqldeveloper-4.0.2.15.21).
1. Remove distfiles (i.e. mv /usr/portage/distfiles/sqldeveloper-4.0.3.16.84-no-jre.zip /tmp).
2. Try to re-emerge =dev-db/sqldeveloper-4.0.3.16.84
3. See the message
emerge -1 =dev-db/sqldeveloper-4.0.3.16.84

These are the packages that would be merged, in order:

[ebuild   RF  ~] dev-db/sqldeveloper-4.0.3.16.84::local_hdd  USE="-mssql -mysql (-sybase)" 231,223 kB

 * 	Oracle SQL Developer for other platforms
 * 		sqldeveloper-4.0.3.16.84-no-jre.zip
 * and move it to /usr/portage/distfiles

4. Wonder a prompt at the end of error message:

Would you like to merge these packages? [Yes/No] 

At this point we already can detect fatal error (no distfiles found and no one can be fetched by portage).

So emerge --ask should fail here, without prompting anything and gathering following errors.

After a large dependency calculation, if I find myself in this situation, then I will let it wait at the --ask prompt until I have manually fetched the files in a separate terminal (using `emerge --fetch --nodeps <atom>` and following the instructions). After I finish fetching the required files, I type 'y' at the waiting --ask prompt, and it happily continues.
