I buy my lenovo x220i notebook (sandybridge platform) from 2011.6 and keep my gentoo up-to-date since then, but this problem kept appearing. I believe the problem is in graphic driver, since it never appear on my other computers, nor in windows on the same machine.
I have never figured out how to trigger this problem, and never find any related error message in syslog or dmesg, but it appeared with every application, no matter in gnome-shell or gnome 3 fallback-mode

please see my screenshot.

and once happends, it keeps appearing, you need to restart to get rid of it. very annoying when you watching video or doing presentation ...
it seams more easy to reproduce when using gnome-shell.

the problem seemed disappeared after I upgraded to xf86-video-intel-2.19.0 (now 2.20.x) and kernel 3.3.x/3.4.x

Thanks, closing then, as the versions you cite to have fixed the problem are also stable by now
