Bug 102536 – URLs within description of [url] tags break the bbcode

If you have something like this:

[url=https://forums.gentoo.org/viewtopic.php?t=366265]can i put gentoo in
www.keeptell.com ? pls tell me how ..[/url].

it gets converted to:

<a href="https://forums.gentoo.org/viewtopic.php?t=366265" target="_blank"
class="postlink">can i put gentoo in <a href="http://www.keeptell.com"
target="_blank">www.keeptell.com</a> ? pls tell me how ..</a>

Need to disable the url matching if it is part of a [url=] tag description,
possibly need change the order that the regexes are run in.
