Since udev 152 "udevadm trigger" no longer requests "add" events by default. See http://git.kernel.org/?p=linux/hotplug/udev.git;a=commit;h=236fae6cf1a619a92174efdf84cd7d91e7d4348d If you have installed >=net-wireless/bluez-4.63, change udevadm trigger --subsystem-match=bluetooth to udevadm trigger --subsystem-match=bluetooth --action=add in /etc/init.d/bluetooth and reboot.
