gpgme needs gnupg for s/mime support, but doesn't ensure that gnupg iself is built with s/mime support. Since it doesn't force any further dependencies, I'd say dropping the smine flag and building gpgsm always is the best way to go. 

Wrong is at the moment that gpgsm-gencert.sh gets installed despite USE=-smime.
