The -2.2.1 ebuild recently had a patch added to allow a user to explicitly call out whether qt4 or qt5 is to be used during the build (otherwise if both were installed, qt5 would be used regardless of USE flags). The change required both a patch (qt4-select.patch) and two updates to the ebuild: the first to use the patch, and the second to enable explicit use flags, i.e. --enable-qt=4 instead of just --enable-qt. It looks like the second change to the ebuild was missed. I will work with a Gentoo developer to get portage updated. In the meantime, if you'd like to patch the ebuild yourself, I've attached the patch.
Created attachment 402406 [details, diff] qt_select_for_vlc-2.2.2.1.ebuild.patch Just to be clear, this is a patch to the ebuild itself so throwing it into /etc/portage/patches will not work.
I just removed all my qt5 libraries (leaving qt4) and confirmed your bug report. I then patched my ebuild using the attached patch and successfully built vlc so as soon as the ebuild is modified we'll be good.
