Steps to Reproduce:
1. preinstalled app-emulation/qemu-1.2.2-r3
2. run 
3. emerge -uDNav world or any other emerge command to update qemu
4. compile is ok but install fails
Actual Results:  
app-emulation/qemu-1.4.0 update fails due to file collisions

Expected Results:  
app-emulation/qemu-1.4.0 update completes without file collisions errors

Fwiw, I have the same problem.

libcacard is a dependency of app-emulation/spice[smartcard] and net-misc/spice-gtk[smartcard]. I recompiled these with USE=-smartcard and removed libcacard to work around this problem.
