dev-lang/ruby-1.9.3_p392 fails to build using the following USE flags:

[ebuild     U ] dev-lang/ruby-1.9.3_p392 [1.9.3_p194-r1] USE="berkdb gdbm ipv6 ncurses rdoc readline ssl yaml -debug -doc -examples -rubytests -socks5 -tk -xemacs"

I removed it altogether. I'm 100% sure that the first two sections in the patch are now applied (although in a different way) upstream. The last two sections were harder to verify, but it looked like there were also changes there to fix parallel make. The approach taken there was quite different, so it was hard to fully verify.

But that patch was for mkmf. Is that also being used when compiling the extensions?

It is.
And as you can see, it's not fixed. Yes I know they merged Daniel's stupid patch before, it's still not fixed.
