When you run current stable util-linux-2.16.1 on < linux-2.6.19[*] you will get during boot:
 * Activating (possible) swap ...swapon: /proc/swaps: unexpected file format
                                         [ ok ]

or you can trigger it later like this:
# swapoff -a; swapon -a
/proc/swaps: unexpected file format

This has been fixed in util-linux-2.16.2 (see commit 7f1a50... in linked changelog) which is already in portage. Stabilizing it will fix the bug, so please stable sys-apps/util-linux-2.16.2 to get rid of this.

[*] there could be many people running these "older" kernels, for example if you use the Xen hypervisor  the official supported Dom0 kernel is 2.6.18.8 and will likely be for the whole Xen-3.x.x life
