squid digest-auth-helpers limited to "password" backend only

Reproducible: Always

Steps to Reproduce:
1. USE="ldap" emerge net-proxy/squid
2. add "auth_param digest program /usr/libexec/squid/digest_ldap_auth ..." to squid.conf


Actual Results:  
/usr/libexec/squid/digest_ldap_auth missing

Expected Results:  
working digest_ldap_auth helper

Fixed in versions 3.0.20 and 3.1.0.15_beta.
