I updated pulseaudio to 0.9.20-r50 and now have very silent sound on my intel hda soundcard. When I downgraded it to 0.9.19-r51 all works well. Me too. A little more info: the volume up/down with the gnome pulseaudio volume applet and gnome keyboard shortcuts don't effect a mixer device. I don't know what they are doing: they show a volume going up/down, and mute, but I does not affect any mixer I can see with alsamixer (not master, not pcm, nothing). I can use alsamixer to change the volume, but it won't go very loud with this version of pulseaudio.
