In it's test-suite, coreutils seems to mount a one-file tmpfs for testing purposes. The relevant line in /proc/mounts (the tests where run in chroot, which is hopefully not relevant):
shm ${CHROOT_PATH}/var/tmp/portage/sys-apps/coreutils-7.2/work/coreutils-7.2/tests/cu-one-file-system.NgZ4CqDklZ/a/b\040(deleted) tmpfs rw,nosuid,nodev,noexec 0 0

At the end of FEATURES="test" emerge -1 coreutils, the mount is still active, and the following message results:

 * Make sure you run 'hash -r' in your active shells

successfully cleans up the mess.

FEATURES="test -sandbox" emerge -1 coreutils does not exhibit this behavior.

none of the tests were marked as failed, so that's weird ...

can you grep the test logs for related errors ?  look for "umount" or similar ?  or try emerging with VERBOSE=yes and post that log file ...

Yes, the lack of test failures seemed quite odd to me as well.
grep seems utterly useless (no useful references to mount, grep one build.log doesn't yield anything referring to the one file system except the two PASSes which use it).

I'll produce a VERBOSE=yes log in a bit, but I also reproduced on a stable x86 machine.
Steps:
ACCEPT_KEYWORDS='~x86' emerge -1 sandbox

same failure. These two configurations share very little, as you might guess (kernel and portage come to mind)

The umount failure seems to happen in tests/rm/one-file-system (log attached).
VERBOSE="yes" and VERBOSE=1 both yielded identical build logs.

you have a sane /etc/mtab right ?  or it's symlinked to /proc/mounts ?  it may be related to you running this in a chroot.  can you try running the sequence of commands manually in the chroot ?  if it works, try running `sandbox` first and then run the commands.

+ mount --bind /dev/shm/tmp6279 a/b
+ cat

Note: the x86 failure above was not chroot, so this bug is robust with respect to chroot. /etc/mtab is sane (/dev/ROOT / ext3 rw,bind,noatime 0 0)

It looks like your suggestion solved it; it also gives me a reason to whine about my favorite pet peeve.
Portage puts / into ADD_PREDICT for src_test, making test failures things very hard to debug, when a sandbox violation would be easy to debug.

mount --bind /dev/shm/tmp6279 a/b tries to access /etc/mtab which obviously fails in the sandbox, causing the later umount failure.

Further testing the working theory, removing the addpredict from ebuild.sh does indeed result in a sandbox violation as expected. I must be missing something however, because if the problem is if I have presented it, shouldn't it have been noticed long ago (and be reproducible for you)?

i dont really know the history behind the / in predict for src_test ... it does seem wrong to me as well, but i never pursued it

i think the reason so very few people are seeing this is because userpriv and usersandbox is set in FEATURES via the profile now (including my case).  that means src_test is run as non-root and so these test cases always get skipped.  i guess putting `addwrite /etc/mtab` into the top of src_test of coreutils should solve the issue ?

I set userpriv/usersandbox for my real system, but they are only default for the dev profile (luckily I am way to lazy to customize a chroot used for testing).
adding 'addwrite /etc/mtab #265725' does in fact fix the problem, and sounds sane so long as coreutils doesn't try to do anything dumb to a relatively critical config file.
