The latest version of nvidia-settings in portage is the beta version 173.08; however the latest version of nvidia-settings in portage is 171.05. On my 8800GTS 512Mb graphics card, using this version of nvidia-settings causes certain options to become missing. One such option is the ability to choose which display device to sync to vblank on (monitor and tv using twinview). I downloaded the source from the nvidia website, built it and ran it instead and my Xvideo options have returned. 
I did not have this issue with a 7600GT card, it would appear it only affects 8xxx series cards (possibly 9xxx also). 

Reproducible: Always

Steps to Reproduce:
1. 8xxx series card (possibly includes 9xxx)
2. Install latest nvidia-drivers and nvidia-settings from within portage
3. Try select a device to sync to vblank on.

I can confirm this. I had the same problem with my GeForce 8600M GT. Installing nvidia-settings-173.08 from source fixed the problem.

I first tried to install it through portage by making a copy of the nvidia-settings-171.05.ebuild and renaming it, but that didn't work (build failed because of some imake issue), so I got the latest source tarball from Nvidia's site and installed that.

Try this ebuild.

I don't have this hardware, so I cannot test it.

Looks like they don't use Imakefiles anymore.

This ebuild works fine for me :)
