Kernel: 2.6.16-r9
Hardware: Adaptec SCSI RAID 2015S
Driver: i2o
Yesterday i have upgrade my kernel from 2.6.14 to 2.6.16 and filesystem totally crashed. 
I investiagate this problem and found next article:
